JOB SUMMARY

 

Directs and coordinates the integration of Case Management functions into the patient care, discharge, and home planning processed with other hospital departments, external service organizations, agencies, and health care facilities; provides direct care to selected patients as appropriate; ensures compliance with quality patient care and regulatory standards.

 

ESSENTIAL FUNCTIONS

 

Coordinate the provision of clinical care to patients, families and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from health care services.

 

Complete psychosocial assessment on all new admissions based on hospital policy and procedures.

 

Develop and participates in the implementation of programs to assist patients and their families with the social and psychological factors of illness.

 

Develop with physicians, nursing personnel, and other members of the management team the planning of patient care services and the patient care plan.

 

Engage in individual or group intervention sessions pertaining to care and adjustment.

 

Arrange for discharge and post-hospital care of patients through institutions and agencies within the community.

 

Prepare statistical reports on department services and patient needs.

 

Develop and maintain harmonious working relationships with other departments; address interdepartmental social service issues.

 

Assist in the preparation of and monitor the department budget in compliance with established budgeting procedures; identify and respond to variances.

 

Develop departmental policies and procedures.

 

Develop hospital policies in relation to community agencies.

 

Assist in planning cooperative projects with other department heads and community agencies.

 

Provide advice and guidance in handling special cases or patient needs.

 

Direct the quality improvement activities for the department and promote the maintenance of high-quality, effective patient care.

 

Direct and coordinate the patient discharge and home-care planning process to facilitate the continuity of care and the provision of effective patient and family support services.

 

Coordinate the provision of information and guidance to patients, their families, and other hospital departments regarding all aspects of patients’ post-discharge treatment and recovery programs.

 

Coordinate the referral of patients and families to appropriate community, social service, home health services, extended care and rehabilitation facilities and organizations.

 

Coordinate Case Management communications to ensure the staff’s awareness of available services and resources; serve as the primary liaison with external social service organizations.

 

Participate in the coordination of interdepartmental program planning and implementation efforts, and support related projects and activities.

 

Participate in the Case Management educational programs for physicians and other departments to enhance their understanding of medical necessity aspects of health care delivery.

 

Coordinate the completion of the Family Satisfaction Survey.

 

Represent the department on various hospital committees.

 

Coordinate and implement Volunteer Program between Host hospital and the Hospital.

 

Develop and implement Pastoral Care Program in conjunction with Pastoral Care from Host hospital.

 

Participate in the Hazardous Material Program.  Hazards include exposure to blood and body fluids, possible communicable diseases, sharp objects and instruments, assorted chemicals and gasses as listed in the Hazardous Materials Program Manual.

 

Comply with measures for preventing exposure to blood borne pathogens.

 

Adhere to Hospital’s attendance policy, as set out in the Employee Handbook.

 

Adhere to all components of the Hospital Compliance Plan in performing job duties and report any violations or suspected violations of the Plan to the Compliance Officer.

 

Demonstrate professional conduct and comply with hospital and departmental policies and procedures.

 

Perform other duties as assigned or delegated by the Director of Clinical Services.

 

Develop an understanding of responsibilities for recognizing abuse and follow policy for making appropriate referrals/interventions.

 

Develop an understanding of responsibilities for Performance Improvement activities and participate in such activities.      

 

Comply with established Safety and Patient Safety Program practices.

MANDATORY QUALIFICATIONS (SKILLS, EXPERIENCE, EDUCATION)

 

Registered Nurse with current New Jersey license

 

Bachelor of Science in Nursing

 

Excellent communication skills

 

Strong organizational skills

 

Computer literate

 

Able to work in high stress environment

 

Able to speak, read and write English

 

 

DESIRED QUALIFICATIONS

 

Masters Degree preferred.

Management experience preferred

 

Minimum two (2) years’ work experience in a hospital preferred.

Minimum one (1) year experience in discharge planning from hospital preferred.

Organizational and planning skills to prioritize work and handle multiple tasks simultaneously

Deal with difficult individuals while maintaining composure

Self-motivated, self directed and well organized

Excellent problem-solving skills
